Glucophage- Level 3

What is it?
 Glucophage- used for Diabetes to lower Blood Glucose
with Type 2 Diabetes (Non-Insulin Dependent Diabetes)
 Available- Tablets 500, 850 and 1,000 mg.; dose should be
adjusted on response and Blood Glucose Levels
o Maximum Dosage- 2,550 mg./day
o Pediatric Patient- 500 mg./day divided doses with
meals
o Taken as- a Pill by Mouth
Side Effects:
 Symptoms- Hypoglycemia/Low Blood Glucose, Nausea,
Vomiting, Anorexia, Heartburn, Diarrhea, Allergic Skin

 See your Health Care Provider- as needed
 See your Pharmacist- for help
 AVOID- if CHF/Congestive Heart Failure, Fever, Trauma,
Severe Infections, Major Surgery, Ketosis, Coma, Acidosis;
 Tablets- DO NOT Crush, Chew, or Cut
 Pregnant- AVOID Using
 AVOID- Alcohol
